Improved layout for report creation

The screens for creating a report have been revised for all report types (CCF, PCF, LCA and ECF) and now use the full available screen width. Input fields and tables are displayed more clearly and are easier to work with. A detailed description on how to create a report can be found in the respective articles for CCF, PCF and LCA.
